Medication Access Program
Qualified health clinic patients will meet the following eligibility requirements:
I. No health care insurance coverage
II. Reside in service area: Boon, Cadillac, Evart, Falmouth, Harrietta, Lake City, LeRoy, Luther, Manton, Marion, McBain, Merritt, Mesick & Tustin.
III. Meet income guidelines: Maximum Annual Income (250% of Federal Poverty Level)

To determine eligibility, please call 231-876-6150 to make an appointment.
Information needed at time of call:
- Household income from previous year.
- Number of Dependents
- Current wages and part time or full time hours.
- If you have insurance or not.
These documents are required at time of appointment:
- Driver’s License or State Identification Card
- Previous year tax return
- One (1) month of current check stubs or unemployment documents.

For the Medication Assistance Program (MAP), recipients can call (231) 876-6151 make an appointment to determine their specific eligibility.
Medication Access qualifications are determined on the following:
- Meet specific pharmaceutical company guidelines.
- Reside in a local service area (listed above).
- Have a primary care provider.
